Independent Clinical Reviews
Independent Clinical Reviews (ICR) are conducted to ensure that care packages commissioned by Health or Social Care are not only clinically appropriate but also deeply person-centred and responsive to individual needs. These reviews are grounded in a holistic approach that considers the whole person, acknowledging their unique strengths, preferences, and aspirations.
What Does LEAF Offer?
At LEAF, I offer a comprehensive and compassionate approach to Independent Clinical Reviews (ICR), grounded in my experience as a specialist Learning Disabilities Nurse Consultant. Each assessment is carried out independently, using a combination of extensive clinical knowledge, up-to-date best practice guidance, and a deep understanding of the lived experiences of people with learning disabilities and their families.
Using a holistic and person-centred framework, every review is designed not just to assess clinical appropriateness, but to see the whole person behind the care package – valuing their voice, choices, and individuality.
Depending on the needs outlined within the commission, my service can include:
Development of a New, Person-Centred Care Plan
I work closely with individuals, their families, and support networks to create tailored care plans that reflect the person's own aspirations, strengths, health needs, and preferences, ensuring care is both empowering and meaningful.
Updated Risk Assessments
Through careful and sensitive risk assessment, I help ensure that risks are identified and managed in a way that promotes safety without limiting independence or quality of life. My approach balances protection with the promotion of autonomy, dignity, and choice.
Tailored Recommendations and Practical Guidance
Every recommendation I make is specific to the individual’s situation and aimed at enhancing quality of life, promoting health and wellbeing, and supporting service providers to deliver outstanding, evidence-based care. I offer clear, actionable guidance to help commissioners, providers, and families plan effectively for the future, whether this involves transition planning, service development, or enhancing current support arrangements.
