Supporting Your Journey: Care and Treatment Reviews (CTRs/CETRs)
As a Learning Disability Nurse Consultant, I advocate for and guide individuals with learning disabilities, offering expert clinical knowledge and championing person-centered care through collaborative partnerships. Central to this are CTRs and CETRs, which ensure care plans are effective, needs are met, and crises are prevented, prioritising open dialogue and individual involvement. These reviews are vital for responsive, adaptable care, fostering positive change and ensuring high-quality support. My goal is to provide comprehensive support and advocacy, ensuring that you or your loved one receives the highest quality, person-centered care. Please contact me if you have any questions or would like to learn more about CTRs and CETRs.
What are CTRs and CETRs?
Person-Centered Approach: Both CTRs and CETRs are designed to be collaborative meetings, focusing on the individual's voice and preferences. We work together to ensure your care and treatment are aligned with your goals and aspirations.
Holistic Assessment: We consider all aspects of your well-being, including physical health, mental health, social needs, and personal development.
Empowering Individuals: These reviews provide an opportunity to discuss your experiences, raise concerns, and actively participate in planning your care.
Who are they for?
CETRs: Specifically for children and young people with learning disabilities.
CTRs: Designed for adults with learning disabilities.
Preventing and Supporting Hospital Admissions: CTRs and CETRs are particularly important for individuals who may be at risk of hospital admission or who are currently receiving care in a specialist learning disability or mental health hospital.
Why are they important?
To ensure your care plan is effective and responsive to your evolving needs.
To promote your safety and well-being.
To help prevent unnecessary hospital admissions.
To ensure that if hospital admission is required, that it is for the shortest time possible, and that all needs are met.
